Greater Glasgow and Clyde Moving Forward Together Programme
The way health and social care services in West Dunbartonshire and NHS Greater Glasgow and Clyde are provided is changing. Managers, clinicians and planners are working together and are engaging with patients, service users and staff to develop plans for a better health and social care system for people.
This new system of care;
Should be organised in the most effective way to provide safe, effective and sustainable care to meet the current and future needs of the population.
It should be designed to support people to improve their own health, manage their own long term conditions and live independently at home for longer.
People should be able to stay in their communities and access primary and community care services close to home.
High quality hospital based care should be available when the required level of care or treatment cannot be provided in the community.

We want to explain why we need to change, our direction of travel and begin describe what our new models of health and social care might look like. Most of all we want to have early conversations and hear what matters most to people.
This is an opportunity to comment and ask questions about how services move forward. Tea and coffee will be provided.
